The Provider Scheduler performs a wide variety of clerical duties for the Medical Services Department for hospice and palliative care.

Responsibilities:
- Schedules patient visits for providers in the electronic medical record. Schedules and creates Patient Tasks upon request, from the Clinical Team Managers, RN Case Managers (RNCMs), admission staff, LVNs, and MSWs. May include coordinating patient visits with Clinical Team Managers, RNCMs, LVNs and MSWs as requested.
- Coordinates urgent visit requests per the direction of the Clinical Manager, Nurse Practitioner Supervisor and/or Chief Medical Officer or designee.
- Schedules Face-to-Face (F2F) visits upon admission as requested by the Access Center.
- Runs the F2F Report twice per month, schedules the F2F visits and sets the 60 and 90 Day recertification tasks.
- Ensures that patients, who reside in SNFs, have monthly visit by a physician, where allowed, a nurse practitioner.
- Confirms patient visits by calling patients and/or responsible parties, documenting calls/confirmation of visit in the electronic medical record and emailing providers and Clinical Team of provider visit.
- Coordinates with the clinical team to ensure that providers receive visit information as requested prior to scheduled visits.
- Responsible for scheduling changes, updating changes in the electronic medical record and notifying the provider (NP/Physician and patient (if requested) of change.
- Responsible for receiving incoming calls from providers, addressing needs of providers, and exercising judgment as to urgency, nature, and confidentiality of the call.
- Performs various other clerical duties in support of Medical Services, such as, scanning, faxing, and copying.
- May request additional clinical documentation from community resources, as requested by hospice provider or clinical team.
- Provides back up support for departmental needs. This includes cross training with other support assistants.
- High school diploma or equivalent.
- Minimum experience of 2-3 years in clerical/office setting: healthcare field required.
- Possess strong phone etiquette/customer service skills, including professionalism, active listening, and speech clarity.
- Ability to handle complaints and difficult situations.
- Demonstrates ability to communicate effectively with staff, management, patients, physicians and community agencies, both verbally and written.
- Possess strong organizational skills, including attention to detail.
- Ability to manage time efficiently.
- Bi-lingual, Spanish speaking preferred.
