Mario Capecchi Drive Salary Range: $22.24 to $35.27 Category: MEDICAL Agency: Dept of Health Application Period: 08/16/2013 â€" 09/16/2013 Part/Full Time: Part Time! Job Characteristics: The pediatric physical therapist (PT) will provide developmental evaluations for infants to young adults and develop individual treatment/care plans for Children with Special Health Care Needs. This will include Neonatal Followâ€"up clinics and itinerant clinics held in different parts of the state. PT will evaluate need for assistive technology and be able to write letters of medical necessity (LMN).
Candidates must have knowledge of principles, theories, and practices of a developmental pediatric PT; particularly geared towards children who were born prematurely. The Physical Therapist must: work closely with a multidisciplinary team at all clinics; maintain effective working relationships with staff, families, medical home, specialists, early intervention programs, and other agencies/providers; maintain an active PT licensure in the State of Utah; comply with policies/procedures for the Utah Department of Health and CSHCN; and prepare writt! en reports of evaluations done on an Electronic Medical Record! (EMR).
