PEDIATRIC NURSE - LPN OR RN
HOURS ARE SUNDAY 3PM TO 10PM AND MONDAY 8 HOURS APPX 10AM TO 8PM
Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.
Essential Duties and Responsibilities:
Work under the supervision of a Registered Nurse.
Provide direct patient care as defined by the state Nurse Practice Act and provides services in accordance with agency policies.
Assist the Registered N! urse/Case Manager in carrying out the plan of care, including ! the assessment of needs; formulation and review of patient care plans.
Perform nursing care according to the needs of the patient and as indicated in the written plan of care.
Recognize and use opportunities to teach/counsel health concepts to the patient and family regarding medication, treatment regiments and self-care techniques/activities. Leave written teaching aides in home.
Have a good understand of Medicaid guidelines.

Qualifications and Experience
Graduate of an accredited practical nurse or vocational nursing program. (Diploma or transcripts to be provided to the agency)
Current license to practice in the operating state as a LPN
Prior home healthcare experience preferred but not necessary
A current TB test, CPR card, driver’s license, and auto insurance must be kept on file
A ! criminal background check must be performed before hire
Valid state driver’s license and reliable automobile
