Davis Hospital and Medical Center opened in 1976 and has been providing high-qua! lity healthcare in Layton for over 30 years now. Before the hospital was built, residents of Davis County had to travel to Ogden or Salt Lake City for hospital services. The hospital has constructed a three-story tower addition. The $36.2 million expansion includes 67 additional beds, a brand new 14-bed Intensive Care Unit, a Women's Surgical Services Unit, and expanded pediatric, medical/surgical and other inpatient services. The expansion will include a conversion of semi-private hospital rooms to private suites.

Social Worker/Discharge Planner
Full time, mostly days.
May work call shifts and weekends.
Responsible for providing social work/discharge planning functions.
Provides resources for inpatients and outpatients, ER, and Rehab.
Performs psycho-social assessments.
Requirements
Bachelor's degree in Social Work.
Social work training in assessment of needs and coordination of services for discharge planning.
Licensed by the State of Utah as a Social Worker. SSW, CSW or LCSW.
Must be able to communicate by written and oral.
Must be able to operate multi-line telephones, computers, fax, pagers, etc.
Must be able to function as a member of a multi-disciplinary team.
Preferences
Prefer 2 years of Social Work experience in a hospital setting.
